Major types of Surgeries performed are:
Endoscopic resection: The procedure involves resection of the tumour through an endoscope in early stage cancer patients where the chance of cancer spread is very low.
Subtotal (partial) gastrectomy: The procedure involves removal of only cancerous part of the stomach along with part of the esophagus or the first part of the small intestine. Most of the times, surgeon also removes the lymph nodes and nearby organs to remove maximum cancer tissue. Subtotal gastrectomy is performed in case of cancer of lower or upper part of the stomach.
Total gastrectomy: The procedure involves total removal of the stomach and nearby lymph nodes along with removal of spleen and parts of the esophagus, intestines, pancreas, and other nearby organs in some cases. Total gastrectomy is performed in case of cancer that has spread throughout the stomach.
The choice of surgery depends upon the uncompromising principles of oncology for complete removal of the cancerous part resulting in longer survival rates of the patients.
Palliative surgery for cancer that cannot be removed
In case of unresectable stomach cancer, surgery is performed either to control the cancer or to relieve cancer symptoms. The surgeries include the following:
Subtotal gastrectomy: The procedure is done to remove cancerous part to treat problems such as bleeding, pain, or blockage in the stomach.
Gastric bypass or Gastrojejunostomy: Large sized tumors can block food from leaving the stomach. Gastric bypass is the procedure in which jejunum is linked to the upper part of the stomach to allow food to leave the stomach through this new connection.
Endoscopic tumor ablation: This procedure is performed in patients who are not healthy enough for surgery with the help of an endoscope to destroy parts of the tumor. This stops bleeding and relieves blockage without surgery.
Stent placement: The procedure involves placing of a stent with the help of endoscope to prevent blockage of the opening at the beginning or end of the stomach, so as to allow food to pass through it.
Feeding tube placement: The cancer patients who become unable to eat or drink enough require placement of a feeding tube through the skin of the abdomen so that liquid nutrition can be put directly into the tube.
